We accept UnitedHealthcare dental PPO plans, whether yours comes through an employer or you bought it for yourself or your family. We also accept AARP Medicare plans from UnitedHealthcare that include dental benefits.
Once you call, the insurance work is ours. We contact UnitedHealthcare, find out what your plan pays toward the care you need, and send the claim after your visit, so you never have to sit on hold with your plan yourself.

What Happens When You Call With a UnitedHealthcare Plan
Three steps take you from a phone call to a finished claim, and only the first one needs anything from you.
1. One Call With Your Member ID
Read us the details on your UnitedHealthcare card when you call or book. We check your coverage with UnitedHealthcare, including your deductible and how much of this year's maximum is still available. There is no charge for the check.
2. Your Estimate Comes First
Before any treatment starts, you get an estimate of your share based on your UnitedHealthcare benefits. From there, the choice of what to do is yours.
3. UnitedHealthcare Gets the Claim From Us
When the visit is over, our team submits the claim to UnitedHealthcare. There is no claim form for you to fill out or mail.
How UnitedHealthcare PPO Plans Usually Sort Your Care
UnitedHealthcare PPO plans commonly put dental care into three groups: preventive, basic and major. The group a treatment falls into shapes how much of the cost the plan picks up. The ranges below are typical, and your benefits check swaps them for the numbers on your own plan.
Cleanings and ExamsPreventive care. Commonly paid in full with no deductible, often twice a year.
Tooth-Colored FillingsBasic care. Often paid at about 80 percent once the deductible is met.
Dental CrownsMajor care. Often paid at about half after the deductible, though some plans pay more.
Dental ImplantsUsually major care on plans that include them, often paid at about half after the deductible. Some plans leave implants out.
Emergency VisitsTreatment to relieve sudden pain is often paid like basic care, at about 80 percent. A filling, crown or other repair that follows is paid in its own group.
Yearly Maximum, Deductible and Waiting PeriodsA deductible of around $50 a person is common, and the yearly maximum is the most your plan pays in one plan year. Plans you buy on your own can make you wait before they cover major work such as implants.

- Start from where you are today. Your first visit looks at your teeth as they are now, and nobody asks you to explain the gap.
- Hear the plan before anything starts. Your dentist tells you what they found, what your options are and why they recommend one, and you can ask as many questions as you like.
- Tell us what makes you nervous. Let us know what worries you before treatment begins, and we will check in with you as we go and take it one step at a time.

By the time you arrive, the insurance questions are settled. In the chair, your dentist looks at your teeth, talks you through what they see, and answers whatever you want to ask.
Bring These With You
- Your UnitedHealthcare member ID card (if your dental coverage has its own card, bring that one)
- A photo ID
- The policyholder's name and date of birth, if the plan is in a spouse's or parent's name
Perfect teeth are not a requirement, and neither is a reason for the gap since your last visit. Any treatment is something you and your dentist decide on together, at a pace that suits you.

Spouses and Kids on Your Plan
Family members covered on your UnitedHealthcare plan are welcome here too, children and adults alike. Have everyone's name and birth date ready when you phone, and we can look up the whole household in a single call.
Many PPO plans pair a deductible for each person with a family deductible. When the family's combined payments reach the family amount, the rest of the family is done with deductibles until the plan year starts over.
AARP Medicare Plans From UnitedHealthcare
We accept AARP Medicare plans from UnitedHealthcare that include dental benefits. Many of them give you a yearly dental allowance that covers preventive care such as exams and cleanings and helps pay toward larger work like fillings and crowns.

Use What Is Left of This Year's UnitedHealthcare Benefits
Many UnitedHealthcare dental plans run on the calendar year, so the yearly maximum starts over on January 1, and some employer plans reset on a different date. On many plans, money you did not use does not carry over, though some roll part of it into the next year.
If a cleaning is due, or your dentist has already recommended a filling or crown, booking it before your plan resets lets this year's benefits pay toward it. A free benefits check tells you how much of this year's maximum you still have.

When Your Care Costs More Than UnitedHealthcare Pays
- After UnitedHealthcare has paid out your yearly maximum, the plan stops contributing until your new plan year begins.

Does UnitedHealthcare cover dental implants?
Many UnitedHealthcare PPO plans do, as a major service, often paying around half the cost after the deductible and up to the yearly maximum. Most plans that include implants also limit how often one can be placed on the same tooth, and plans bought on your own may make you wait first. Is a Golden Rule dental plan a UnitedHealthcare plan?
It can be. Golden Rule Insurance Company underwrites some of UnitedHealthcare's individual and family dental plans. If your card or paperwork names Golden Rule, call us with it and we will check whether it is a UnitedHealthcare PPO we accept.
Do I need a referral to see a dentist with a UnitedHealthcare PPO?
No. A dental PPO lets you choose your dentist, so you can book with us directly without a referral from another provider.
Does UnitedHealthcare pay for cosmetic dental work?
Work done only to change how teeth look is usually not covered. Treatment that repairs a damaged tooth and improves its appearance at the same time, such as a crown on a cracked front tooth, may be partly covered as restorative care. We check your benefits before we quote any cosmetic work.
Can I use my UnitedHealthcare plan for a dental emergency?
Yes. Your plan applies to an urgent visit the same way it applies to any other. Call (602) 866-8183 before noon on a day we are open, Monday through Thursday or Saturday, and we will do our best to see you that day.
