Have your card ready
Keep your insurance card and member ID in front of you. If the plan isn't in your name, you'll also need the plan holder's name and date of birth.
Call or book online
Call (602) 866‑8183 or book through Zocdoc, and tell us which insurance you have. We'll take it from there.
Come in for your visit
Come to our office on Bell Road. Your dentist explains what they found and your options, and you decide what happens next. Questions are welcome.

What Happens to Unused Dental Benefits?
Most dental plans run on the calendar year and start over on January 1. If you still have benefits left, here's what to know. Want the bigger picture? See what dental insurance usually covers.
- Your yearly maximumThis is the most your insurance pays toward your dental care in a year.
- Unused benefitsWhatever you don't use by December 31 usually doesn't roll over to next year.
- Your deductibleIf you've paid toward your deductible this year, that usually starts over in January too.
- Your plan datesNot every plan follows the calendar year. Check your insurance paperwork for your dates, or ask us when you call.

What If You Don't Have Dental Insurance?
- You don't need dental insurance to be seen here.
- No insurance? New patients can get a $19 exam and X-rays, which covers the exam and X-rays only. It can't be combined with insurance.
- We accept CareCredit, a healthcare credit card for paying over time, subject to approval. You can prequalify with no credit impact.
- We also accept dental savings plans, such as Careington. A savings plan isn't insurance: you pay to join and get lower prices at offices that accept it.
Questions About Your Insurance
Can I see any dentist with a PPO plan?
Most PPO plans let you choose your own dentist, so you can use yours at Diamondback Dentistry in North Phoenix without switching plans. How much your plan pays for each visit still depends on the plan itself and on how much of this year's benefits you've already used.
Do you accept every plan from the insurance companies you list?
For each insurance company in our list, we accept its PPO plans at every plan level, whether you have a basic plan or a higher one. The one exception is Delta Dental: we accept Delta Dental PPO plans, but not DeltaCare USA, which is a DHMO plan.
What if my plan isn't listed?
Call us anyway at (602) 866-8183. We file claims with out-of-network plans too. With an out-of-network plan, you pay at your visit and we send the claim to your insurance company. What they cover depends on your plan.
Are PPO plans good for dental?
A PPO plan is often a good fit if you want to choose your own dentist, since HMO and DHMO plans usually limit you to a set list of offices. How much does a dental PPO cost?
It depends on the plan and on whether you get it through your job or buy it on your own. Most PPO plans have a monthly premium, a deductible you pay first and a share of the bill you pay after that. Your plan documents or your employer's benefits team can give you the exact numbers.
